Intraspinal Epidermoid Associated with Granuloma


Abstract
Intraspinal epidermoids, involving the thoracic spinal cord are quite rare. On reviewing the literature, a few cases of the thoracic intraspinal epidermoids are reported.
The vast majority of intraspinal epidermoids are commonly associated with a dermal sinus and occur usually in the lumbosacral segments.
T.I. Tan presented intradural and extradural epidermoids.
Recently we experienced a spinal extradural epidermoid in the thoracic cord.
A 22 year old man was admitted to Capital Armed Forces General Hospital with 8 months history of dorsal pain and with 3 days of history of paraparesis. A lumbar puncture revealed normal manometrics and total protein contents of 63mg% in the cerebrospinal fluid. The thoracic spine films revealed kyphosis with compressed fracture of T5 and erosion of pedicles and body of T4-5-6. Thoracic myelogram showed blockade at the level of lower margin of T5. Laminectomy of T4-5-6-7 was performed, and noted extradural granuloma on the posterolateral region of the right side and cystic epidermoid on the ventrolateral region of the left.
We are going to report a extremely rare case of extradural cystic epidermoid at the thoracic cord.
